Hello and welcome, new patients!

The Little Sprouts Team is excited to meet you! Before you come in, there is some important information you should know.

Before your first appointment…

We ask that every new patient fills out four different forms, all found on our “Patient Forms” page. These documents include Dental and Medical History, General Agreement and Consent, Financial Policy Form, and Patient Information Form. If you want to streamline your visit to Little Sprouts, you can print and fill out these four documents and either email us a completed copy or bring it with you to your appointment.

Our “Patient Forms” tab also includes a Patient Bill of Rights & Responsibilities and a Privacy Notice, which are there for your information. We encourage you to read these documents, but we don’t need a signed copy.

At Little Sprouts Dental, children’s health, safety, and comfort are our highest priorities. When you come to your appointment, we will ask to scan a copy of your ID and insurance card. For the first visit, we strongly suggest that the parent accompanies the child. If you would like a family member to bring your child to Little Sprouts at a later date, we will provide you with a Consent to Release Protected Health Information form.

At your appointment…

The first visit is always what we call a “look-and-see.” We will clean and examine your child’s teeth, and take x-rays depending on the age of the patient. We offer fluoride, which is painlessly brushed on to teeth, in order to strengthen and protect them from decay. For more information, please see our Flouride Page. We will also go through our Caries (Cavity) Risk Assessment process with you, and we will discuss any treatment recommendations and options.

After your appointment…

The front desk will be happy to schedule your child’s next appointment. Each patient will receive a “Goodie Bag,” full of the essentials of good oral hygiene, and they will be given their choice from our ever-changing Prize Box!
